Noticing hot flushes, missed periods or unexplained anxiety at 40 can be unsettling — especially when friends the same age seem unaffected. Menopausal transition at 40 is earlier than average but far from rare, and in some cases it signals early menopause that genuinely needs treatment, not just reassurance. The Vesey Private Hospital in Sutton Coldfield offers 30-minute women’s health GP appointments and same-week hormone blood testing, 7 days a week, with no referral needed.

The average UK age for menopause (your final period) is 51, and most women start perimenopause in their mid-40s. But around 1 in 20 women go through menopause between 40 and 45 — called early menopause — and about 1 in 100 before 40, known as premature ovarian insufficiency (POI).

At 40, symptoms are more likely to be early perimenopause: the years of fluctuating hormones before periods stop. That distinction matters, because the approach to testing and treatment is different — and because menopause before 45 carries long-term health implications that make treatment more important, not less.

Signs of Menopause and Perimenopause at 40

Period changes are usually first: cycles becoming shorter (21–24 days), heavier, lighter or skipping altogether. Hot flushes and night sweats — sudden waves of heat, often with a racing heart. Sleep problems — 3am waking is characteristic. Mood changes — new anxiety, irritability or low mood, often mistaken for burnout or depression. Brain fog, joint aches, headaches, vaginal dryness, low libido and weight gain around the middle complete the familiar pattern.

At 40 these symptoms overlap heavily with thyroid disease, iron deficiency and stress, so proper assessment matters more than at 50. Don’t accept — or self-apply — the label of “just stress” without basic tests.

Why Testing Matters More Under 45

For women over 45, guidelines advise diagnosing perimenopause on symptoms alone. Under 45, blood tests are recommended: two FSH levels taken 4–6 weeks apart help confirm early menopause or POI, alongside thyroid function, ferritin, B12 and vitamin D to exclude mimics.

Confirming early menopause isn’t academic. Losing oestrogen a decade early raises the long-term risk of osteoporosis and cardiovascular disease, so NICE guidance recommends women with early menopause or POI take HRT (or the combined pill) at least until the natural age of menopause — for most, the benefits here clearly outweigh the risks. Fertility is also a live question at 40, and an AMH test alongside FSH gives a fuller picture of ovarian reserve if you’re weighing options.

At The Vesey, hormone profiles are taken in clinic with results in 24–48 hours and reviewed with a GP rather than posted to you as raw numbers. Individual tests start from £32.

What Happens Next

If early perimenopause is confirmed, treatment is tailored: body-identical HRT for symptoms and long-term protection, contraception advice if needed (HRT is not a contraceptive), bone health planning, and follow-up to adjust doses. If tests point elsewhere — thyroid, iron, B12 — treating that cause often resolves the symptoms entirely.

Frequently Asked Questions

Can menopause really start at 40?

Yes. Around 5% of women reach menopause between 40 and 45 (early menopause), and symptoms of the perimenopausal transition can begin several years before that. Symptoms at 40 always warrant proper assessment rather than reassurance alone.

What tests confirm early menopause?

Under 45, two FSH blood tests taken 4–6 weeks apart are recommended, plus thyroid function, ferritin, B12 and vitamin D to exclude conditions that mimic menopause. AMH can add information about ovarian reserve, particularly if fertility is a consideration.

Is HRT safe if I'm only 40?

For women with early menopause, HRT is not just safe but recommended until at least the natural menopause age of 51, to protect bones and heart. It is effectively replacing hormones your body would normally still be producing.

How quickly can I get tested at The Vesey?

Same-week — usually within 24–48 hours for the blood draw, with results back in 1–2 working days and a GP appointment to review them. No referral is needed and the clinic is open 7 days a week, 8am to 8pm.
