Menopause and HRT — getting it right
Around 13 million UK women are currently peri- or post-menopausal. NHS menopause specialist services are limited; most women are managed by their GP, with variable confidence and time.
Our BMS-accredited menopause clinic offers:
- Full symptom assessment using validated scoring (Greene Climacteric Scale) including the under-discussed urogenital, musculoskeletal, mood and cognitive symptoms
- Individualised HRT prescribing — including body-identical oestrogen (gel, patch or spray), micronised progesterone (Utrogestan), local vaginal oestrogen, and testosterone for low libido where appropriate
- Cardiovascular and bone risk assessment at start of HRT
- Routine review at 3 months and annually
- Honest discussion of risks — including breast cancer risk (small but real), VTE risk (very low with transdermal oestrogen), and the limitations of current evidence
We also see women for whom HRT is the wrong answer, or who want non-hormonal management — and provide that with the same care.
Heavy menstrual bleeding (HMB) — the structured approach
Around 1 in 5 women describe their periods as heavy. NICE recommends a structured approach:
- History and examination — including pelvic examination and assessment for anaemia
- Investigations — FBC, ferritin; ultrasound if anatomical cause suspected; endometrial sampling if over 45 or with risk factors
- Initial management — tranexamic acid, NSAIDs, hormonal contraception or LNG-IUS (Mirena coil)
- Specialist review if first-line management fails, with options including endometrial ablation or hysterectomy

Can I see a female consultant?
Yes. Female gynaecologists and our female menopause specialist are routinely available. Please specify when booking.
Will the appointment include an examination?
If clinically appropriate. Pelvic and speculum examinations are discussed with you in detail beforehand. A female chaperone is offered routinely for any intimate examination; you can also bring a friend or family member.
How quickly can I have a cervical smear?
Usually within 48–72 hours. The smear is sent to an accredited NHS cytology lab — results are typically available in 7–10 days with a written report.
What's included in the menopause consultation?
A 45-minute appointment with our BMS-accredited menopause specialist: full symptom review, cardiovascular and bone risk assessment, discussion of HRT and non-hormonal options, prescriptions issued same day, and a clear written plan.
Will you write to my GP about HRT?
Yes with consent. A detailed letter is sent within 48 hours so your NHS GP can take over prescribing if appropriate (some practices accept private menopause specialist initiation; others require their own assessment first).
Can you remove or replace a coil?
Yes — coil removal £140, coil insertion (Mirena IUS inclusive of device) £480. We perform both copper and hormonal coils.
Do you treat endometriosis?
We assess and offer first- and second-line medical management (analgesia, hormonal options). Where surgery is needed — diagnostic laparoscopy, excision of endometriosis — we refer to specialist endometriosis surgeons with continued co-management.
Fertility and AMH — what do you offer?
AMH blood test (ovarian reserve) £95. Pelvic ultrasound and antral follicle count. Full preconception advice and PCOS workup. We're not a fertility clinic — IVF and ICSI require referral to a licensed HFEA centre, which we coordinate.
Does insurance cover gynaecology?
Bupa, Vitality, AXA Health, Aviva and most major insurers cover gynaecology. Cervical screening for routine screening may not be covered (it's an NHS programme). Pre-authorisation always advisable.
