What it is
Gynaecological cancers include cancers of the womb (endometrial), ovary, cervix, vulva and vagina. The clinical bar for investigation is deliberately low — most patients we see do not have cancer, but the women who do benefit enormously from early diagnosis. Our pathway exists to bring NHS-standard rapid-access workup forward by weeks.
Symptoms and signs
- Post-menopausal bleeding — any bleeding more than 12 months after your last period.
- Persistent post-coital or inter-menstrual bleeding in pre-menopausal women.
- Persistent abdominal bloating, early satiety, pelvic discomfort or unexplained weight loss.
- A new pelvic mass or abnormal smear / HPV result requiring colposcopy.
- A visible vulval lesion that is itchy, sore or changing.
How we investigate
Same-week consultant assessment, transvaginal pelvic ultrasound, CA-125 and other tumour markers where indicated, outpatient hysteroscopy with directed endometrial biopsy for abnormal uterine bleeding, colposcopy for cervical concerns, and onward MRI / CT scanning through partner imaging centres. Where a cancer is confirmed, we hand over to a tertiary oncology MDT — but we get you to that MDT faster.

Should I be seen urgently for post-menopausal bleeding? +
Yes. Any bleeding more than 12 months after your last period must be investigated promptly. While the majority of cases have a benign cause (atrophic endometrium, endometrial polyp), post-menopausal bleeding is the cardinal symptom of endometrial cancer and requires an ultrasound and often hysteroscopy.
Which gynaecological cancer is most common? +
Endometrial (womb) cancer is the most common gynaecological cancer in the UK. It is also the most detectable early, because it almost always causes abnormal bleeding before it spreads — which is why we treat any unexplained bleeding as urgent.
What tests will I need at the first appointment? +
A consultant examination, transvaginal ultrasound, and blood tests including CA-125 and HE4 where ovarian cancer is a concern. If the ultrasound suggests womb lining thickening or a cavity lesion, outpatient hysteroscopy with biopsy is arranged the same week.
What happens if gynaecological cancer is confirmed? +
We refer you directly, consultant-to-consultant, to a tertiary gynaecological oncology MDT — with a named-pathway letter, not just a standard referral. We manage the insurance pre-authorisation and remain your point of contact for queries during the waiting period.
