What it is

"Menstrual disorder" is an umbrella term for heavy menstrual bleeding (menorrhagia), painful periods (dysmenorrhoea), absent periods (amenorrhoea), infrequent or unpredictable bleeding (oligomenorrhoea), and intermenstrual or post-coital bleeding. Common causes include fibroids, polyps, adenomyosis, endometriosis, PCOS, thyroid dysfunction, IUD-related issues, perimenopausal hormonal change and (rarely) endometrial or cervical pathology.

Symptoms and signs

  • Periods so heavy you flood through protection or pass large clots.
  • Period pain that requires regular days off work or routine opioid analgesia.
  • Periods that have stopped without explanation (other than pregnancy, breastfeeding or menopause).
  • Bleeding between periods, after intercourse, or after the menopause — always worth investigating.
  • Cycles longer than 35 days or shorter than 21 days, persistently.

How we investigate

Initial consultation includes a structured menstrual history and pelvic examination. Investigations typically include a hormone panel (FSH, LH, oestradiol, prolactin, TSH, AMH where relevant), pelvic ultrasound and — for abnormal bleeding patterns — outpatient hysteroscopy with directed endometrial sampling.

Treatment options at The Vesey

  • Mirena IUS — first-line for heavy menstrual bleeding without distorted anatomy; fitted on-site.
  • Combined hormonal contraception — for cycle regulation and dysmenorrhoea.
  • Tranexamic acid and mefenamic acid — non-hormonal options for heavy bleeding.
  • Polyp / fibroid management — see-and-treat outpatient hysteroscopy where structural cause is found.
  • Endometrial ablation — for women whose family is complete and bleeding is the dominant issue.
  • Targeted treatment of underlying cause — PCOS, thyroid dysfunction or perimenopausal change managed in their own right.
What causes heavy menstrual bleeding? +

Common causes include fibroids, endometrial polyps, adenomyosis, endometriosis, PCOS, thyroid dysfunction and perimenopausal hormonal change. In some women no structural cause is found (dysfunctional uterine bleeding). Investigation guides treatment.

Can a hysteroscopy be done at The Vesey? +

Yes. We offer outpatient hysteroscopy with directed endometrial sampling on site. Most procedures take 10–15 minutes with local anaesthetic and do not require a general anaesthetic.

Can the Mirena IUS be fitted at The Vesey? +

Yes. The Mirena IUS is fitted on site by our consultant gynaecologist. It is first-line treatment for heavy menstrual bleeding in women without distorted uterine anatomy.

Is period pain ever a sign of endometriosis? +

Dysmenorrhoea severe enough to require regular analgesia or time off work is a red flag for endometriosis. Other features include deep dyspareunia, cyclical bowel or bladder symptoms, and subfertility. Laparoscopy is needed for a definitive diagnosis.
