Miscarriage
What is a miscarriage? A miscarriage is the loss of a pregnancy during the first 23 weeks. They are fairly common with 1 in 8 pregnancies ending in miscarriage. Most people who experience a miscarriage go on to have successful pregnancies. Losing 3 or more pregnancies in a row (recurrent miscarriages) is uncommon and only affects around 1 in 100 women. What are the symptoms of a miscarr
What it is
Sporadic miscarriage is common and usually has no identifiable cause. Recurrent miscarriage (RCOG threshold: three consecutive losses, or two with risk factors) deserves a structured workup that may identify a treatable cause — antiphospholipid syndrome, parental chromosomal translocation, uterine anomaly, thrombophilia, thyroid dysfunction, poorly-controlled diabetes, severe vitamin D deficiency, or unexplained.
Symptoms and signs
- Recent miscarriage with unresolved grief, anxiety, or relationship strain.
- Two or more consecutive miscarriages.
- Miscarriage at any gestation where the cause is unclear.
- Pre-conception planning after pregnancy loss.
- Coexisting subfertility — combined miscarriage and fertility assessment.
How we investigate
Detailed history, antiphospholipid antibody screen (lupus anticoagulant, anticardiolipin, anti-beta2-glycoprotein), thrombophilia screen in selected cases, thyroid function, HbA1c, vitamin D, parental karyotype where indicated, pelvic ultrasound and 3D / saline hysterosonography or hysteroscopy for uterine anomaly.
Treatment options at The Vesey
- Evidence-based reassurance after sporadic loss — most subsequent pregnancies are successful.
- Antiphospholipid syndrome treatment — low-dose aspirin and low-molecular-weight heparin in confirmed cases.
- Uterine anomaly correction — hysteroscopic septum resection where appropriate.
- Thyroid optimisation, glycaemic optimisation, vitamin D replacement.
- Genetic counselling for chromosomal translocation carriers.
- Early-pregnancy reassurance scanning with our gynaecology team in the next pregnancy.
How many miscarriages qualify for a recurrent miscarriage investigation? +
RCOG guidelines recommend investigation after three consecutive pregnancy losses, or after two if there are additional risk factors such as maternal age over 35, known uterine anomaly, or thrombophilia. We offer early referral for two losses where clinical concern exists.
Can antiphospholipid syndrome be treated to prevent future miscarriage? +
Yes. Confirmed antiphospholipid syndrome is treated with low-dose aspirin started preconception and low-molecular-weight heparin from a positive pregnancy test. This treatment reduces miscarriage risk substantially in confirmed cases.
Is early-pregnancy reassurance scanning available at The Vesey? +
Yes. We offer early-pregnancy reassurance scanning from 6 weeks gestation. These scans are particularly valuable after recurrent miscarriage to confirm viability and location, and to provide psychological reassurance in the anxious first trimester.
How long should we wait before trying to conceive after a miscarriage? +
Evidence does not support mandatory waiting after a single first-trimester miscarriage. After investigation for recurrent miscarriage, timing depends on the result: for APS, anticoagulation should be optimised first; for anatomical causes, surgical correction may be advisable before the next pregnancy.
