What it is

ARFID is significant food restriction (number, volume or category of foods accepted) plus one of: faltering growth, nutritional deficiency, dependence on supplements or marked psychosocial impact. In autistic and sensory-processing-different children, sensory-driven selectivity is the most common driver. Treatment is multidisciplinary, sensitive and slow.

When this is relevant

  • Severe food selectivity beyond typical fussy eating.
  • Sensory-led food refusal - texture, smell, appearance, brand specificity.
  • Gagging, vomiting or anxiety on attempted food introduction.
  • Faltering growth, micronutrient deficiency, dependence on a small number of "safe" foods.
  • Significant family mealtime stress, school day food refusal.
  • Coexisting autism, ADHD or sensory processing differences.

How we investigate

Structured feeding history, growth and developmental review, sensory and autism-spectrum screening, micronutrient bloods (FBC, ferritin, vitamin D, B12, folate, zinc), coeliac screen.

What is the difference between ARFID and ordinary fussy eating? +

Fussy eating is common in toddlers and usually resolves without intervention. ARFID is distinguished by its severity and impact: faltering growth, nutritional deficiency, need for supplements, or significant psychosocial impairment. ARFID persists into adolescence and adulthood without treatment.

At what age can ARFID be diagnosed? +

ARFID can be diagnosed from early childhood through to adulthood. In practice, most children are referred around school age when the impact on nutrition and social function becomes clear. There is no minimum age criterion.

Do neurodiverse children with ARFID always need tube feeding? +

No. Tube feeding is reserved for children with severe malnutrition or those unable to meet nutritional needs orally. Most children with ARFID are managed with gradual food exposure, sensory-integration work, micronutrient supplementation and family support without enteral feeding.

How does sensory processing affect eating in autistic children? +

Many autistic children have heightened sensitivity to food texture, smell, appearance, temperature or the mealtime environment. This can cause gagging, vomiting or extreme distress on food introduction. A specialist paediatric dietician experienced in autism-related feeding difficulties develops a personalised low-pressure food-exposure plan.
