What it is
Most MSK pain is from common mechanical causes (osteoarthritis, tendinopathy, muscle strain, ligament injury, bursitis) and responds well to structured rehabilitation. Inflammatory causes (rheumatoid arthritis, spondyloarthritis, polymyalgia rheumatica), referred pain, and red-flag presentations (infection, fracture, malignancy) are screened at every consultation.
Symptoms and signs
- Localised joint pain worse with use, better with rest (degenerative).
- Stiffness lasting more than 30 minutes in the morning, with joint swelling (inflammatory).
- Pain reproduced by specific movements or resisted tendon loading (tendinopathy).
- Pain referred from another structure — hip pain from the back, shoulder pain from the neck.
- Constant pain not eased by rest, night pain, unexplained weight loss — needs urgent workup.
How we investigate
History, examination, focused bloods (FBC, CRP, ESR, rheumatoid factor, anti-CCP, urate where indicated). X-ray, ultrasound and MRI through partner imaging centres. Joint aspiration and analysis where appropriate.

What is the difference between mechanical and inflammatory joint pain? +
Mechanical pain is worse with use and better with rest. Inflammatory pain is worse after rest and in the morning, improves with movement, and may be accompanied by joint swelling, warmth and fatigue. Blood markers (CRP, ESR, RF, anti-CCP) help distinguish the two.
Are image-guided injections available at The Vesey? +
Yes. Dr Arul James performs ultrasound-guided injections for joints, bursae and tendons on site. Fluoroscopic guidance is used for spinal and deep joint procedures. Common targets include shoulder, hip, knee, ankle and facet joints.
When should I see a pain specialist rather than a physiotherapist? +
Consider a pain specialist if physiotherapy has failed after 6–8 weeks, if the diagnosis is unclear, if you have inflammatory markers or red-flag features, or if you need an image-guided injection that physiotherapists cannot provide.
Can ultrasound-guided injections help knee osteoarthritis? +
Yes. Ultrasound-guided corticosteroid injections provide short-term pain relief. Hyaluronic acid injections offer longer-term benefit for mild-to-moderate OA. Genicular nerve radiofrequency lesioning provides sustained relief for those who respond to diagnostic nerve blocks.
