Procedures and treatment pathways

Pain medicine is now multidisciplinary by design. The Vesey offers the full menu of evidence-based interventional procedures, infusion pathways, and medication optimisation — alongside the psychology, physiotherapy and occupational support that make those treatments stick. Every pathway is led by Dr Arul James (Consultant in Pain Medicine, GMC Specialist Register).

Spinal & joint injections

Facet joint injections, medial branch blocks, nerve root blocks, epidural steroid injections (interlaminar, transforaminal and caudal), sacroiliac joint injections, intra-articular hip / knee / shoulder injections (corticosteroid or hyaluronic acid). Performed in our procedure suite under image guidance where indicated.

Radiofrequency procedures

Standard (thermal) radiofrequency ablation for proven facet-mediated pain. Pulsed radiofrequency (PRF) as a non-ablative alternative for dorsal root ganglia and peripheral nerves where standard RFA carries motor risk. Cooled RFA for sacroiliac joint denervation. Six- to twelve-month benefit is typical.

IV lidocaine infusion pathway

Our flagship neuropathic pain pathway — free 30-minute pain review with a Health Advisor (reviewed by Dr James), then if eligible a £260 consultation, then a monitored IV lidocaine infusion within two weeks for £540. Full pathway →

Ketamine infusion (selected cases)

Sub-anaesthetic IV ketamine for complex regional pain syndrome, refractory neuropathic pain, and selected chronic-pain-with-depression presentations. Patient selection is careful — assessment, BP/ECG screen, and cardiology sign-off where appropriate. Joint care with your existing mental-health team.

Botulinum toxin (BoNT-A) for chronic migraine

NICE-approved PREEMPT protocol for chronic migraine (≥ 15 headache days / month) failing two or more preventatives. Quarterly injections, headache-diary monitoring. Insured patients welcome — BUPA, Vitality and AXA cover this for documented chronic migraine.

Neuromodulation referrals

Where indicated we facilitate referral for spinal cord stimulation (SCS), peripheral nerve stimulation (PNS), and intrathecal drug delivery system assessment with NHS-tertiary or our partner private neuromodulation centres. We continue the medication optimisation and rehabilitation work alongside the implant pathway.

Medication optimisation

Gabapentinoid, tricyclic, SNRI and opioid review — including supported deprescribing for patients on long-standing or escalating regimens. We never simply add another medicine; the assessment always asks whether existing therapy is still doing what it was supposed to.

Diagnostic imaging

Same-week MRI, ultrasound and X-ray pathways through our partner imaging centres. We will not perform an interventional procedure on a presumed diagnosis — imaging that confirms the target structure is part of every procedural plan.

Multidisciplinary pathway

Chronic pain is rarely fixed by a single procedure. We work alongside physiotherapy, clinical psychology / CBT and occupational therapy partners — every patient who would benefit from non-pharmacological care leaves their initial consultation with a clear plan and warm-handover referrals. For complex presentations we coordinate a written MDT note with your NHS GP so primary care stays in the loop.

When pain medicine is not the right specialty

We are honest about the limits of our clinic. We will refer onward (with a letter) if your presentation is acute red-flag back pain, suspected cauda equina, vascular claudication mistaken for sciatica, or a primary rheumatological condition that needs an inflammatory disease workup before procedural intervention. Pain medicine works best when it is the right tool for the right problem.
