What it is

The pouch arises through a weak area between two parts of the cricopharyngeus muscle. Diagnosis is by barium swallow or endoscopy. Modern endoscopic stapling divides the wall between the pouch and the oesophagus, eliminating food trapping in a 30-minute day-case procedure.

When this is relevant

  • Regurgitation of undigested food, sometimes hours after eating.
  • Halitosis (chronic bad breath) despite oral hygiene.
  • Chronic cough, hoarseness or recurrent chest infections from aspiration.
  • Feeling of food sticking, slow swallowing, weight loss.
  • Audible gurgling on swallowing.

Barium swallow (gold standard for pharyngeal pouch), flexible nasendoscopy, endoscopic ultrasound where indicated.

What does the endoscopic stapling procedure involve? +

The Dohlman procedure is a day-case operation under general anaesthetic. A rigid endoscope is passed through the mouth and a surgical stapler divides the common wall between the pouch and the oesophagus, eliminating the food trap. Most patients eat a soft diet the following day and go home the same day.

How long is recovery after pharyngeal pouch surgery? +

Most patients return to a normal diet within one to two weeks. Swallowing typically improves noticeably within days of the procedure. Driving is usually possible 24–48 hours after the anaesthetic has cleared.

Can a pharyngeal pouch come back after surgery? +

Recurrence is possible, particularly with smaller pouches, and occurs in roughly 5–10% of cases over many years. A repeat endoscopic stapling is usually straightforward and carries the same low-risk profile as the first procedure.

Are there alternatives to surgery for a pharyngeal pouch? +

For patients unfit for general anaesthetic, conservative strategies — chin-tuck swallowing technique, smaller meals, avoiding lying down after eating and head-of-bed elevation — can reduce regurgitation. Surgery remains the only definitive treatment.
