What it is

Tonsillectomy removes the palatine tonsils. The dominant indications in adults are recurrent tonsillitis (typically ≥ 7 episodes in 1 year, ≥ 5 per year for 2 years, or ≥ 3 per year for 3 years per SIGN criteria), peritonsillar abscess, obstructive sleep apnoea, and suspected tonsil malignancy. Modern technique uses cold-steel or coblation under day-case general anaesthetic.

Symptoms and signs

  • Recurrent severe tonsillitis interfering with work, school or daily life.
  • Chronic sore throat, halitosis and tonsil stones causing quality-of-life impact.
  • Single previous peritonsillar abscess (quinsy) — usually an indication.
  • Loud snoring and witnessed apnoea associated with tonsil hypertrophy.
  • Asymmetrical tonsils, persistent unilateral sore throat, or visible lesion — needs assessment for tonsil malignancy.

How we investigate

ENT examination including fibreoptic nasendoscopy where appropriate. Documentation of episode frequency. Sleep study where OSA is in the picture. Biopsy or tonsillectomy where any suspicious lesion is identified.

  • Structured 14-day recovery plan — paracetamol and ibuprofen regimen, hydration, post-operative diet, return-to-work guidance.
  • Bleeding-risk counselling — primary haemorrhage (< 24 hours, < 1%), secondary haemorrhage (day 5–10, 2–5%) and the exact escalation path if it happens.
  • Coblation tonsillectomy — selected centres, often less post-operative pain at the price of slightly higher delayed-bleed risk.
  • Alternative pathway counselling — for borderline indications, a structured watchful-waiting plan with PPI trial, dietary review and infection log.
How painful is recovery from adult tonsillectomy? +

Recovery is typically more painful in adults than in children. Most patients require regular paracetamol and ibuprofen for 10–14 days. Pain often peaks around day 5–7 as the post-operative slough separates. Keeping well hydrated and eating regularly (even when it hurts) is the best thing you can do to aid healing. We provide a structured written recovery plan at discharge.

When is tonsillectomy indicated according to guidelines? +

UK SIGN guidelines typically support tonsillectomy for adults with 7 or more episodes of true tonsillitis in one year, 5 or more per year for two years, or 3 or more per year for three years — provided the episodes are genuinely tonsillitis (fever, exudate, cervical lymphadenopathy or positive strep test) rather than viral sore throats. A previous peritonsillar abscess (quinsy) is usually sufficient on its own.

Can tonsil stones be managed without surgery? +

Many patients manage tonsil stones (tonsilloliths) without surgery using good oral hygiene, gargling with saline, and manual removal with a cotton bud or water flosser. Tonsillectomy is the definitive cure and is appropriate if stones are causing significant halitosis, discomfort, or recurrent infections. At consultation we will assess stone burden and symptom severity to guide the decision.

Will my adenoids be removed at the same time? +

Adenoids shrink naturally after puberty and are rarely a significant contributor to symptoms in adults. Adenoidectomy at the time of adult tonsillectomy is therefore not routinely performed unless examination shows persistent adenoid tissue contributing to nasal obstruction or recurrent sinusitis. Your ENT surgeon will assess this at consultation.
