Trigger Point Injections
A focused injection into the tight, tender knots that develop in muscle — most commonly in the trapezius, paraspinal, gluteal and piriformis muscles — to break the spasm-pain cycle and restore movement.
Led by Dr Arul James (Consultant in Pain Medicine, GMC Specialist Register) at The Vesey, Sutton Coldfield. Self-pay and insurance accepted (BUPA, Vitality, AXA, WPA, Cigna, Aviva, Healix). Same-week appointments typical.
Who this procedure is for
- Myofascial pain syndrome with palpable trigger points reproducing your typical pain pattern.
- Tension-type or cervicogenic headache driven by neck and shoulder trigger points.
- Chronic low back, gluteal or piriformis pain in the absence of nerve root compression.
- Pain that has plateaued on physiotherapy and would benefit from a reset to engage with rehab.
How the procedure is performed
After mapping the trigger points by palpation (and ultrasound where deep muscles are involved), local anaesthetic ± a small dose of steroid is injected directly into each knot. Typically 2–6 points are treated per visit. The procedure takes 15–25 minutes.
Before, during and after
- Before: no fasting required. Arrive in loose-fitting clothing that gives access to the relevant area.
- During: awake, you will feel the needle and a brief reproduction of your usual referred pain — that twitch response confirms a correctly-placed injection.
- After: stretch the muscle gently before leaving; we will demonstrate the post-procedure exercises.
Recovery and aftercare
Drive yourself home. Expect 24–48 hours of muscle soreness similar to a hard workout. Relief usually develops over 3–7 days. Trigger point injections work best when paired with a focused physiotherapy programme — the injection releases the knot, the rehab stops it returning.

Risks and side effects
Common: site soreness, bruising, brief flush. Uncommon: vasovagal reaction, post-injection flare. Rare: infection, pneumothorax (for chest-wall trigger points — minimised with ultrasound), allergic reaction.
When this procedure is not appropriate
Active skin infection over the injection site, severe needle phobia (we offer alternatives), bleeding disorder that cannot be safely managed.
