About the procedure

What is facet joint injection?

Facet joints are small joints at the back of the spine that connect adjacent vertebrae and provide stability and limited spinal movement. Like any joint, they can develop arthritis — most commonly in the lumbar (lower back) and cervical (neck) spine — causing pain that is typically worse with extension and rotation of the spine.

Facet joint injections deliver local anaesthetic and steroid directly into the facet joint under fluoroscopic guidance. They serve a dual purpose: diagnostic (significant pain relief after injection confirms the facet joints as the pain source) and therapeutic (the steroid reduces joint inflammation and provides pain relief). A positive response to facet joint injection also identifies patients suitable for radiofrequency ablation for longer-lasting relief.

Am I a candidate?

Who is facet joint injection for?

You may be a suitable candidate for facet joint injection if you are experiencing any of the following:

  • Chronic axial back or neck pain worsened by extension and rotation
  • Pain not radiating significantly into the arm or leg (distinguishing it from disc-related radicular pain)
  • Spinal pain related to known facet joint arthritis on CT or MRI
  • Exploration of the facet joints as a pain source before considering radiofrequency ablation
  • Failure of physiotherapy and oral analgesia for chronic spinal pain

The process

What happens at The Vesey

From your first consultation to discharge — here is what to expect at every stage.

Consultant assessment

Clinical examination and review of imaging to identify whether the facet joints are likely to be a significant pain source.

Preparation

Aspirin and anticoagulants adjusted if required.

Facet joint injection

Under fluoroscopic guidance, small needles are placed into the facet joints at the implicated levels. Steroid and local anaesthetic are injected. Takes approximately 20–30 minutes.

Post-procedure assessment

Pain scores are recorded before and after the injection to assess the diagnostic response.

After your procedure

Recovery — what to expect

Light activity recommended for 24 hours. Some patients experience temporary increased soreness for 24–48 hours before the steroid effect begins.

Your consultant will provide you with a personalised recovery plan at your assessment appointment, based on your individual procedure, health status, and activity requirements.

Your questions answered

Facet Joint Injection — frequently asked questions

How do I know if my back pain is coming from the facet joints?
Facet joint pain is typically central or paracentral, worsened by extension and rotation, and not usually associated with significant radiating leg pain. Imaging findings alone are not sufficient to confirm the diagnosis — a diagnostic injection with documented pain relief is the most reliable confirmation.
How much does a facet joint injection cost at The Vesey?
Facet joint injection at The Vesey is from £1,200. A pain management consultant assessment costs from £260.
Is facet joint injection the same as an epidural?
No. Facet joint injections target the small posterior joints of the spine. Epidural injections target the epidural space to treat nerve root compression causing radicular pain. Both are performed under image guidance but at different anatomical locations.
How long does facet joint injection pain relief last?
Relief typically lasts 2–6 months. Patients who respond well and consistently are candidates for radiofrequency ablation, which provides longer-lasting relief of 12–24 months.
Do I need a referral for facet joint injection at The Vesey?
No — book a pain management consultant directly. Call 0121 387 3727 or book online.
