What is varicose vein treatment (evla)?
Varicose veins are enlarged, twisted veins most commonly appearing in the legs, caused by damaged or weakened valves within the vein that allow blood to pool rather than flowing efficiently back to the heart. They affect approximately 30% of the adult population and can cause pain, aching, swelling, skin changes, and in advanced cases, leg ulcers.
Endovenous laser ablation (EVLA) is the gold standard minimally invasive treatment for varicose veins. A thin laser fibre is inserted into the affected vein under ultrasound guidance, and laser energy is delivered to close the vein. The procedure is performed under local tumescent anaesthesia and takes approximately 45–60 minutes. NHS treatment for varicose veins is now largely unavailable unless complications are present.

Who is varicose vein treatment (evla) for?
You may be a suitable candidate for varicose vein treatment (evla) if you are experiencing any of the following:
- Visible varicose veins causing pain, aching or heaviness in the legs
- Swelling of the ankles or lower legs associated with varicose veins
- Skin changes — pigmentation, eczema, or lipodermatosclerosis around the ankle
- A previous superficial thrombophlebitis (clot in a varicose vein)
- Venous leg ulcer — active or healed

Consultant assessment and duplex ultrasound
Duplex ultrasound mapping of the venous system to identify the source of reflux and plan treatment.
Pre-operative assessment
Routine health check. Aspirin and anticoagulants may need to be adjusted.
EVLA procedure
Tumescent local anaesthesia is injected around the vein. A laser fibre is inserted under ultrasound guidance and withdrawn slowly, delivering laser energy to close the vein.
Recovery
Discharged the same day. Compression stockings required for 2 weeks.
Recovery — what to expect
Return to light activities and work within 2–3 days. Compression stockings worn for 2 weeks. Avoid strenuous exercise for 2 weeks. Follow-up duplex scan at 6 weeks.
