Japanese encephalitis (JE) is a serious viral infection of the brain caused by the Japanese encephalitis virus, transmitted through the bite of infected Culex mosquitoes. It is the leading cause of viral encephalitis in Asia, with an estimated 68,000 cases and 13,600 to 20,400 deaths occurring annually across 24 countries. While most infections are asymptomatic or mild, approximately 1 in 250 infections results in clinical encephalitis — of which 20 to 30% of cases are fatal, and 30 to 50% of survivors sustain permanent neurological or psychiatric complications.
The vaccine (Ixiaro) provides reliable protection and is recommended for all travellers to endemic areas in Asia and the Pacific who will be spending time in rural or agricultural settings, regardless of trip duration. Risk is highest during the monsoon season and in rice-growing regions adjacent to pig farming.

Vaccination schedule
The Japanese encephalitis vaccine (Ixiaro) is given as a two-dose course, with the second dose administered 28 days after the first. The full course must be completed at least one week before potential exposure. For adults who received the primary course at least one year previously and remain at risk, a single booster dose is recommended before further travel to endemic areas. An accelerated schedule (day 0 and day 7) is available for urgent travel and has been shown to produce similar immunogenicity in adults up to age 65.
