Joint pain (arthralgia) has many causes ranging from mechanical wear-and-tear to autoimmune inflammatory disease. Accurate diagnosis is essential because the treatment for osteoarthritis, rheumatoid arthritis, gout and psoriatic arthritis differ substantially. Early diagnosis of inflammatory arthritis in particular can prevent joint damage and disability.
What it is

Symptoms and signs
- Morning stiffness lasting more than 30 minutes — typical of inflammatory arthritis.
- Hot, swollen, red joint — may indicate gout, septic arthritis or acute flare of inflammatory arthritis.
- Symmetrical small joint pain in hands and feet — suggestive of rheumatoid arthritis.
- Single large joint pain (knee, hip) in older patients with graded onset — typical of osteoarthritis.
- Joint pain associated with skin rash, eye inflammation or bowel symptoms — consider psoriatic arthritis, reactive arthritis or IBD-associated arthropathy.
How we investigate
Blood tests: FBC, CRP, ESR, RF, anti-CCP, ANA, uric acid, HLA-B27. X-ray (joint space narrowing, erosions). Ultrasound (synovitis, effusion, crystal deposition). MRI for soft tissue or early erosive disease. Synovial fluid aspiration and microscopy if acute arthritis.
Treatment options at The Vesey
- NSAIDs and analgesia — symptom control while diagnosis is established.
- Corticosteroid joint injections — rapid anti-inflammatory for acute flares; performed under ultrasound guidance.
- Disease-modifying anti-rheumatic drugs (DMARDs) — methotrexate, hydroxychloroquine, sulfasalazine for inflammatory arthritis.
- Biologic therapies — TNF inhibitors, IL-6 blockers and JAK inhibitors for refractory inflammatory arthritis; initiated and monitored under specialist supervision.
- Orthopaedic referral — direct pathway for joint replacement where conservative management has failed.
