Rheumatoid arthritis: why speed matters
RA is an autoimmune condition causing progressive joint destruction if untreated. The cornerstone of modern management is the treat-to-target strategy — starting DMARD therapy early and titrating until remission or low disease activity is achieved.
The most commonly used first-line DMARD is methotrexate, often combined with hydroxychloroquine and sulfasalazine (triple therapy). The therapeutic window is narrow: studies consistently show that patients who start DMARD therapy within 12 weeks of symptom onset have significantly better long-term outcomes and less radiographic damage.
At The Vesey, Dr Shanmugam can arrange baseline bloods (FBC, LFT, RF, anti-CCP, CRP, ESR, hepatitis serology) same-day and initiate a DMARD prescription within the first appointment where clinically appropriate. Monitoring bloods — FBC and LFTs monthly initially — can be performed in-clinic or at the patient's NHS GP as part of a shared-care agreement.
For patients who do not respond adequately to conventional DMARDs, biologic therapy — including TNF inhibitors (adalimumab, etanercept) and IL-6 receptor antagonists (tocilizumab) — is available via established NHS shared-care protocols. Pre-biologic screening (TB, hepatitis B, IGRA) is arranged at The Vesey.
Gout: more than a diet problem
Gout is the most common inflammatory arthritis in men over 40, caused by the deposition of monosodium urate crystals in joints. Acute attacks — characterised by a red, hot, swollen joint, most often the first metatarsophalangeal joint (base of the big toe) — are exquisitely painful and require urgent treatment.

The target serum uric acid is below 360 μmol/L (lower in patients with tophi).
Many GP-managed patients are significantly undertreated — placed on a low dose of allopurinol without dose titration. Undertreated gout leads to chronic tophaceous gout with permanent joint damage, nephrolithiasis, and chronic renal impairment. Gout is also frequently associated with metabolic syndrome — hypertension, insulin resistance, obesity — and cardiovascular risk should be assessed at the same time.

Severe organ-threatening disease (nephritis, cerebral lupus, antiphospholipid syndrome with thrombosis) is referred to nephrology or immunology as appropriate.
What is DMARD monitoring and why is it needed?
Disease-modifying anti-rheumatic drugs (DMARDs) such as methotrexate, hydroxychloroquine and sulfasalazine require regular blood monitoring — FBC and LFTs monthly initially, reducing to 3-monthly once stable. What is polymyalgia rheumatica (PMR)?
