What is acl reconstruction?

The anterior cruciate ligament (ACL) is one of the four major ligaments of the knee, connecting the femur to the tibia and providing rotational stability. ACL tears are among the most common serious knee injuries, particularly in football, rugby, and skiing. When the ACL ruptures — either partially or completely — the knee becomes unstable, and many patients find it impossible to return to sport without surgical reconstruction.

ACL reconstruction involves replacing the torn ligament with a tendon graft, most commonly taken from the patient's own hamstring (autograft) or from a donor (allograft). The procedure is performed arthroscopically — using small incisions and a camera — and typically takes 60–90 minutes under general anaesthesia.

Consultant assessment

Your consultant orthopaedic surgeon will review your MRI, assess your knee stability, and discuss the surgical options most appropriate for your anatomy and activity level.

Preparation

Pre-operative physiotherapy to strengthen the quadriceps and hamstrings significantly improves surgical outcomes. Your consultant will discuss pre-hab requirements at your assessment.

Reconstruction

Performed under general anaesthesia. The surgeon harvests the graft, prepares bone tunnels in the femur and tibia, and secures the new ligament in the anatomical position of the original ACL.

Rehabilitation

Structured rehabilitation begins within 24 hours of surgery and typically spans 9–12 months before full return to competitive sport. Your care team will provide a detailed rehab protocol.

After your procedure

Recovery — what to expect

Most patients are discharged the same day or after one night. Crutches are required for 2–4 weeks. Return to light activity at 3 months, jogging at 4–6 months, and full sport at 9–12 months, subject to achieving rehabilitation milestones.
