Migraine: the underdiagnosed disabler
Migraine affects 1 in 7 people in the UK, yet 50% have never received a formal diagnosis. Most have been living with what they call "bad headaches" and self-managing with over-the-counter analgesia — often making the problem significantly worse through medication-overuse headache, a condition where frequent analgesic use paradoxically increases headache frequency.

First seizure — what happens next?
A first tonic-clonic seizure requires neurological assessment within 2 weeks as recommended by NICE guideline NG217. At The Vesey, the first-seizure clinic includes a full history (including witness account where available), EEG referral, MRI brain referral, and mandatory DVLA driving cessation advice — a 6-month abstention from driving is legally required after a first unprovoked seizure in the UK.
The neurological assessment determines whether medication should be started immediately — indicated where there is a structural lesion on MRI, an EEG abnormality consistent with epilepsy, or where the seizure was nocturnal — or whether a period of observation is the appropriate management for a single provoked event. A full written DVLA letter is provided at the appointment, explaining the patient's legal obligations clearly and documenting the clinical advice given.
