About this test
Testosterone is the primary male sex hormone, responsible for muscle mass, bone density, libido, mood regulation, and red blood cell production. Levels decline at approximately 1 to 2% per year from age 30 onwards. Clinically significant hypogonadism — symptomatic low testosterone — is estimated to affect 2 to 6% of men, with many more experiencing suboptimal levels with significant quality of life impact. This panel provides comprehensive assessment of bioavailable testosterone, including the calculated free fraction representing the biologically active hormone.
Note: Measures total testosterone, free testosterone (calculated), SHBG, free androgen index (FAI), and albumin.
What this test can detect
- Hypogonadism — primary (testicular) or secondary (pituitary) testosterone deficiency
- Low free testosterone despite normal total testosterone — relevant when SHBG is elevated
- Elevated SHBG — reducing bioavailable testosterone in ageing, liver disease, or hyperthyroidism
- Monitoring of testosterone replacement therapy — ensuring therapeutic range without supraphysiological levels
Who should have this test?
- Men with symptoms of hypogonadism — reduced libido, erectile dysfunction, fatigue, loss of muscle mass, or low mood
- Those already on testosterone replacement therapy (TRT) requiring monitoring
- Men over 40 experiencing significant unexplained fatigue or loss of vitality
- Athletes wishing to monitor androgen status
